This can be done through our secure online portal or by faxing the encounter form or superbill to us. The encounter form or superbill should include the following information:
- Patient demographics: name, date of birth, address, and insurance information.
- Date of service and the location where the service was rendered.
- Procedure or service codes that accurately describe the treatment provided.
- Diagnosis codes that support medical necessity for the treatment provided.
